What the Brain Health Blueprint is

The Brain Health Blueprint is a health assessment and educational consultation. It includes a validated computerized cognitive assessment that you complete on your own, a consultation with Michael K. Lowe, MD, a written plan delivered within two business days of that consultation, and a scheduled follow-up call to review that plan.

What it is not

This distinction matters, and it is the reason this page exists.

This service is not emergency care. If you are experiencing a medical emergency, call 911 or go to your nearest emergency department.

How the assessment works

You complete the assessment remotely, on your own, before your consultation. It is administered through CNS Vital Signs, an independent computerized assessment platform, which scores your results and stores your test data on its own system. Dr. Lowe reviews your results before you speak.

Results are compared against age-matched normative data. Like any test, results can be affected by effort, fatigue, illness, medication, or misunderstanding the instructions. An unexpected result may mean the test should be repeated.

Your information

Your assessment results, the notes from your consultation, and your written plan are retained by Brain Health Neurology. Scheduling and payment are handled by third-party services, and your test data is processed by CNS Vital Signs.

Your information is not sold, and it is not shared with employers, insurers, attorneys, or family members unless you specifically direct it. You may request a copy of your results at any time.

Permission to use de-identified information

This is a separate question, and your answer does not affect anything else.

With your permission, Brain Health Neurology may use information from your assessment and consultation — with your name, contact details, and other direct identifiers removed — for research, publication, and improvement of this service.

Because de-identified information cannot be traced back to you, once it has been de-identified and combined with other records it cannot be located or withdrawn.

You are free to decline. Declining does not change the service you receive, what you pay, or anything about how you are treated.

Payment, rescheduling, and refunds

Payment is due at booking. The rescheduling and refund terms are shown at checkout when you book. This service is not billed to insurance and does not generate insurance-compatible documentation.
